Athletic Health Forms - 2008-09 School Year

The fall season starts August 11, 2008.  

All athletes must have a preparticipation physical to participate in a sport.  The preparticipation physical evaluation for fall sports cannot be performed earlier than 
June 1. 

You may use your own doctor or the school's doctors.  Moravian Academy offers a free sport physical to all athletes for each sports season.  The doctors will be at the Upper School campus on August 11, 2008 .  The doctors will be providing a comprehensive sports history and physical preparticipation screening examination at the school on that day.  The doctors will not be providing a full physical; therefore, testicular exams will not be provided. 

Please download the appropriate forms, print them, fill them out and return them to the Athletic Trainer.  Please return forms for Middle School students to Wendy Sheetz at 11 West Market Street , Bethlehem , PA 18018 .  Please return forms for Upper School students to David Connors at 4313 Green Pond Rd , Bethlehem , PA 18020 .

Please note that the forms required for an athletic physical are not the same forms that the school nurse may send you in the mail for getting a school physical.  We are required to use the PIAA approved forms for athletic physicals.

All forms must be returned to the athletic trainer on or before the first day of practice or your child will not be able to participate.

The first form is for parents and students to complete.    

Middle School students need to download the Parent/Student form.  

Upper school students need to download the Parent/Student/ImPACTForm.   This form includes a parent consent form for the ImPACT concussion management program that we are using with all Upper School athletes. This program will enable us to better evaluate athletes who have suffered a concussion.  You can visit the ImPACT web page at www.impacttest.com to get more information about the software.  In Section 2, please sign across from all sports your child may participate for the entire year.   

The second form is for the physician to complete.  

Moravian Academy will be providing a Sports Physical free of charge on the first day of Fall practice at the Upper School . 

You need to download the Doctor Evaluation Form if you are using your own doctor to complete the physical.
