Health forms are very important and
must be completed and returned to the Academy, each
year, before students may participate in classes or
sports, including
practice.
Students
Participating in Sports are required to have
physicals documented on Pennsylvania
Interscholastic Athletic Association (PIAA) Forms
that are found on the athletics section of this website.
All Students
must have the following general health forms completed
before beginning classes at the end of August.
These forms should be addressed to Mrs. Grencer,
school nurse, and returned by August 1st.

Physician Physical Exam Report Form
- All students new to the Upper School, as well as 11th
graders are required to have a complete current physical
examination.
Additional Forms
that May be Required:
Food Allergy Action Plan
- If your child has an allergy to food, environmental
substance or other product, please complete this form.
Epi Pens must be provided by individuals for
their specific use in emergency situations.
Medication Administration Consent
If any medications are necessary during school hours,
please complete this form and provide the medication to
the nurse in the original pharmacy container.
If your child carries a respiratory inhaler, this
form should be completed as well.
Special
Health Needs
Should your child have any special health needs, please
note them on the Personal
Medical History Form that must be completed for all
students. If
you would like to discuss specific issues or needs, with
the nurse, please call her at 610-691-1600 ext 318.
Mrs. Grencer will then contact you before the
start of the school year. She will also be available to
meet with you personally, should this be helpful, at any
time during the school year.
